Reporting A Rare Species

If you have observed any of the plants or animals on our rare species list, please take the time and complete our rare species sighting form.

Rare Species Sighting Field Form - (PDF format, 25.6KB)

Click here to get acrobat reader (You will need Acrobat Reader to view this file)

If you cannot complete all of the field options you can leave them blank, but please be as accurate as possible with the information you do have. The most important data are detailed location descriptions, dates, and observer name.

Along with the form please send the following data and attachments if possible:

  • The Global Positioning System (GPS) coordinates
  • A photocopy of a topographic map showing the precise location of the occurrence. If you do not have a topo map, you can place a dot on a county road map or gazetteer.
  • Photo of species- some species are tricky to identify and we need a photo or a very detailed description to ensure the accuracy of the record.
  • Your contact information- name, address, phone number and email address so we can contact you with any questions.

Our program relies on the eyes of others. Without the help of state biologists, researchers, and wildlife enthusiasts a tremendous amount of information would go undocumented. These records help us to understand where these species are and how we can help protect them.

Please print out the form and mail to:
